ABSTRACT

@#Objective To explore the factors related to the recovery of nil per os (NPO) patients after stroke by retrospective data analysis, and to establish a predictive model.Methods The information of demographics, evaluation and treatment of 141 stroke patients admitted to the Hearing and Language Department in Beijing Bo'ai Hospital from April, 2017 to November, 2020 were selected. The predictive model was established by univariate analysis and Logistic regression. The fitting degree and discriminant validity of the model were evaluated by Hosmer-Lemeshow (H-L) test and receiver operating characteristic (ROC) curve. Other 121 patients with post-stroke dysphagia from December, 2020 to November, 2021 were used as the validation set to verify the model.Results For univariate analysis, National Institute of Health Stroke Scale (NIHSS) score, drinking water test results, autonomous cough ability, cough after swallowing, movement ability of tongue and jaw, and electrical stimulation treatment were significantly associated with the outcome (H=65.803, χ2 > 4.623, P<0.05). Multivariate Logistic regression analysis showed that NIHSS score (X1, OR=0.772, 95%CI 0.64 to 0.82, P<0.001), spontaneous cough ability (X2, OR=5.116, 95%CI 1.28 to 20.41, P=0.021), and electrical stimulation during treatment (X3, OR=94.718, 95%CI 5.65 to 1589.26, P=0.002) were independent factors for the outcome of swallowing function. Thus, the predictive model was P=11+e−(2.368−0.325X1+1.632X2+4.551X3) P = 1 1 + e - ( 2.368 - 0.325 X 1 + 1.632 X 2 + 4.551 X 3 ) , which was well fitting (P=0.845), with the largest area under curve (0.884). The overall accuracy of the model in the validation set was 91.7%.Conclusion The patients with dysphagia would like to recover well if he/she was with lower NIHSS scores and normal autonomous cough ability; meanwhile, the addition of electrical stimulation therapy in comprehensive rehabilitation may be helpful. A predictive model has been established, which needs a further research.

ABSTRACT

@#Objective To explore the rehabilitation for chorea after bilateral thalamus hemorrhage. Methods A case was reviewed. Re-sults The score of Fugl-Meyer Assessment of balance increased from 0 to 7, the modified Barthel Index increased from 25 to 50, and the co-ordination of movement improved significantly after rehabilitation treatment. Conclusion Early comprehensive rehabilitation may reduce the chorea movement, improve the activities of daily living and balance for chorea after bilateral thalamus hemorrhage.

@#Objective To observe the immediate effects of the sustained forced-blowing exercise (SFBE) on the phonation function in young healthy people. Methods 43 young healthy people were asked to pronounce a longest /a/ in a comfortable situation within one breath for 3 times before and after SFBE. The sound files were analyzed with computer sound analysis system, with the parameters of mean maximum phonation time (MMPT), mean frequency (MF) and mean sound pressure level (MSPL). Results The MMPT and MSPL increased (P< 0.01) after SFBE. There was no significant difference in the MF (P>0.05) before and after SFBE. Conclusion The SFBE can improve the MPT and MSPL of the healthy people obviously, without affections on the MF.

@#As the diagnosis and treatment levels increased, the cure rate and lifetime of the patients with breast cancer improved. In China,surgery and drug treatment received more attention, but the overall rehabilitation was ignored. This article summarized the status of breast cancer patients's rehabilitation in lymphoedema, upper limb dysfunction, psychological problem, sexual dysfunction, childbirth trouble,dystrophy, follow-up visit and family-society support, and so on.

ABSTRACT

Objective To analyze the characteristic of tuberculous encephalitis following with dysphagia. Methods The characteristic of the patient was analyzed, and the signs and symptoms were inconsistent. And it was different with neuro dysphagia. Videofluoroscopic swal-lowing study was used to further examine. Results The cause of the patient's dysphagia was confirmed as tracheoesophageal fistula. Conclu-sion It exists clinically that brain lesion accompanied with constructional dysphagia without neurogenic dysphagia.

ABSTRACT

@#Speech therapy in China, as speech pathology in the other countries, is one of important parts in rehabilitation therapy field. With the development of rehabilitation college education, speech therapy was also set as a common test course in college. This field has its particular characteristics within rehabilitation therapy practice in order to improve the effect of teaching during short period class time in college. This article reviewed the curriculum characteristics and class time allocation of speech therapy course in China college education currently, and summarized the curriculum content and characteristics with the speech therapy course of the rehabilitation therapy profession in Chinese college, and analyzed its class time allocation, methods and practice of the teaching techniques. As a result, the practice course was thought to be very important in order to improve the practice course of speech therapy, and arouse the students' interest and understanding with this part. Five new teaching thoughts were introduced to enhance the practice of speech therapy.

@#ObjectiveTo compare the effect of acupuncture and speech rehabilitation on motor aphasia after stroke with speech rehabilitation alone. Methods49 patients with aphasia were divided into treatment group (27 cases) and control group (22 cases) randomly. The cases in treatment group were treated with acupuncture and speech rehabilitation. The cases in control group were treated with speech rehabilitation alone. They were assessed before and 1 month, 3 months after treatment. ResultsThere was no significant difference between treatment group and control group 1 month after treatment (P>0.05), but was significant in auditory comprehension, speaking, reading, dictation and description (P<0.05) 3 months after treatment. ConclusionThe acupuncture can facilitate the recovery of motor aphas after stroke.

@#Objective To survey the kinetic function of articulating organs in children with cerebral paralysis.Methods The kinetic characteristics of articulating organs in 115 cerebral paralysis children aged 2~6 were observed,including frequency of respiration,stability of respiration,if there was reverse respiration,whether the respiration of mouth and nose was separated,the longest articulating and expiration time,original reflections of mouth and so on.Results and Conclusion The kinetic function of children's articulating organs was limited just like that of their limb.So their kinetic characteristics of articulative organs differed from adults's and normal children's.

@#Objective To explore the feasibility of catheter sacculus expanding therapy for cricopharyngeal achalasia.Methods A patient cricopharyngeal achalasia after cerebral infarction treated with catheter sacculus expanding was reported.Results After 38 times expanding therapy,the patient could eat by himself without choke and whoosh.Video fluoroscopic(VF)showed that the cricopharyngeal muscle can open normally when the food went through.And there is no any aspiration.Conclusion Catheter sacculus expanding therapy can improve cricopharyngeal achalasia effectively,and can be applied easily,safe and reliable.

ABSTRACT

@#ObjectiveTo investigate the reliability and validity of Chinese Rehabilitation Research Center Standard Aphasia Examination (CRRCAE) among patients with aphasia after cerebral injury. Methods40 patients were measured using the CRRCAE. For reliability, interclass correlation coefficients were analyzed. For validity, the correlation coefficients of the aphasia quotients and 4 items of CRRCAE and Western Aphasia Battery (WAB) were analyzed.ResultsThe interclass correlation coefficients>0.9, while the Cronbach α of total scale was 0.9411, the scale of the comprehension was 0.8677,the scale of express was 0.8889. CRRCAE had positive relationship with WAB(with the aphasia quotients, r=0.948,P<0.01). The effect size statistics scale was 0.94.ConclusionCRRCAE shows a good retest reliability and good internal consistency. The total scale of CRRCAE can be correlated with serious level of aphasia effectively.

@#目的探讨交叉性失语患者的的临床表现及康复疗效。方法对 4例交叉性失语症的右利手患者初次发病时的语言病理症状 ,及经过正规的语言康复训练后的改善效果进行归纳分析。结果及结论交叉性失语症多出现于右侧大脑半球因外伤受损的患者中 ,为非流畅性失语 ,言语表现以听理解轻度障碍、命名及复述轻度障碍、阅读理解轻度障碍和表达、自发性书写明显障碍的语言障碍征候群。其介入训练的时间越早越好 ,训练显效时间为 3周左右 ,康复训练效果与其训练前病程相关 ,1年以上训练效果较差。

@#The Chinese Standard Aphasia Examination was made according to Chinese language and cultural habits by referring to Japanese Standard Language Test of Aphasia.151 normal people and patients whose brain damaged without aphasia were tested.Mean and standard deviations were calculated.The significant differences were not found by analysis of variance to age,sex,handedness,profession and education except oral instruction and describing pictures in different educational groups.Therefore,the examination is suitable for Chinese aphasics who live in different parts of China and spoken mandarin.
